I am looking for suggestions for a quilt intake form that can be utilized for customer quilting. 

do you use something like this that you wouldn't mind sharing with me?  thanks, K

Sorry....I haven't gotten to my laptop at all today. The forms I use are on its hard drive. I spent a good portion of the day at the hospital going through some tests. I'll try to send them tomorrow.

Thanks for sharing your intake sheet!  I just spent a few minutes looking over your whole website.  Very nice job!  Nice gallery, too.  I like the descriptions - giving the rationale for choosing design and batting, etc.

I would be interested in a copy of your intake form also.  A friend just asked me for a quote on a quilt she made.  I may need to start some quilting and sewing for others for an income boost, so I want to begin in the right range and process.

if you go to my website, withakquilting.blogspot.com, and click on the quilt intake form, you can see what i am using. this might give you some ideas.

 

Linda, ffq-lar, has a great intake form that i used to generate mine. 

 

beware though, i have not received any business through this site. i am way behind in my posts, and am concentrating on building my etsy shop instead, since longarming isn't doing it for me due to where i live, i think.
