Are you sure you have the needle in the proper direction?  

Most Domestic Sewing Machines (DSM) use a needle that has a flat side on the shaft that faces the rear of the machine.  If you put the needle in backwards, when you tighten the retaining screw it may not hold the needle securely.  Refer to your DSM manual.  If you do not have the manual, the long grove should face where the thread comes down and goes through the "eye" of the needle.
