Since I also make quilts from start to finish (what was I thinking when I started to do this!!!), I can share that also. If anyone is interested in either form, please email me at amdquilts@yahoo.com and I will be happy to reply by attaching a pdf format of either or both forms. My form originated from information provided by both DeLoa and Jessica Jones at a workshop several years ago. It has worked for me and could work for you. PLEASE adjust the pricing to meet the needs of your area. I live and work in a poor county in PA. I charge as much as I can at this point--even though I believe I should be charging more;).
